Please send the attached dispute form along with the receipt obtained either via email as an attachment to your response or alternatively via fax on the following number: 17865133980 along with the following details:
1. The institution name and address.
2. Institution’s telephone number.
3. The amount, time and date of the transaction.
4. A scanned copy of a receipt obtained at the time of the request.
5. The last eight digits of your Globe Wallet account number.
6. The attached signed dispute form.
